Evidence based care bundle programme to reduce falls. Applicable in all health care settings. Initially developed and introduced by the Royal College of Physicians, supported by the Health Foundation. Extension project carried out in acute setting. Dementia known to be an independent factor for falls risk.
Outcome
Project identified the need for integrated, multi-disciplinary approach to falls prevention. Carefully implemented, FallSafe reduces falls by 25%. Our programme did not have the resources of the original, but we did see a reduction of between 6-13% on average
