The part delivered by Southampton primary care Ltd is seeing residents and offering comprehensive geriatric assessments for each resident, medication reviews, long term condition reviews, sign posting to other services. Our team compromises a ANP, GP, HCA, Physiotherapist, OT and Dietician.
