Description
Community neuropsychology service - accepting GP and health professional referrals for adults of working age with an acquired brain injury who live in Portsmouth or South East Hampshire.
Referral Criteria
Referral Criteria:
Referrals can be made by Acute and Post Acute Hospital Services, community neuro services (including the Community Stroke Rehabilitation Team and other neuro rehab health professionals), GPs, brain injury case managers, Adult Mental Health services and other health professionals. Where the GP is not the referrer, we may contact them for further information when considering the suitability of the referral.
The service accepts referrals for patients who have received a diagnosis of a non-progressive form of acquired brain injury, and who are:
Adults of working age
Diagnosed with acquired brain injury as the primary condition.
Living in their own home and not resident in a nursinig or care home.
Not actively abusing alcohol or other substances.
