Description
Solent Neurological Rehabilitation Service is comprised of:
1. Snowdon _ a 14 bed neuro- rehabilitation ward.
2. Snowdon @Home - specialist neurological early supported discharge team.
Specialist longer-term community neuro-rehab teams
1. CNRT-community based neuro-rehabilitation therapy
2. Vocational Rehabilitation Team- encouraging enablement to some form of work (Occupational therapy)
3. Rehabilitation gym-out-patient neuro-rehabilitation 1:1 and groups(Physiotherapy)
4. Transition team-supporting 14-25 years in move from paediatric to adult neuro services
5. Spasticity & Intra-thecal Baclofen services ( in conjunction with UHS)
Referral Criteria
Catchment area: Adults (18+) with a neurological diagnosis who are registered with a Southampton City GP.
Referral process:
Self-referrals accepted. Patients with non-organic/functional neurological symptoms will be seen after diagnosis by a neurologist.
Waiting lists: The team operates urgent and routine waiting lists. Patients agreed as urgent are offered an appointment within two weeks. Patients on the routine waiting list will be contacted with the outcome of the triage within five working days including an estimate of the current waiting list length in weeks-maximum 18 weeks.
